Client Grievance - Written Acknowledgement

It is the policy of Changing My Direction, LLC to respect and uphold the rights and

grievances of clients served. Client advocates for grievances are Braylene Adams (CEO) and Kia Hurley (Clinical Supervisor). A thorough investigation of the grievance will be conducted, which may also include contact with the Ohio Mental Health and

Addiction Services, Ohio Civil Rights Commission, the ADAMHS Board, and/or the U.S. Department of Health and Human Services.
